Things to bring when come to clinic: Insurance certificate, (a list of current medication, medical referral letter, and your dentures if you have).

If your teeth have been broken or knocked out, especially by trauma, bring them. Pulled out teeth can possibly be fixed if they are under kept under wet conditions in saliva or milk. In that situation, please come to our clinic immediately.
